Community Care Programs

FHC – Family Health Center
(through Marshfield Clinic)

A federally funded program designed to provide preventive and primary health care to persons or families with limited income resources. Provides a variety of services for those who are eligible and qualify. Must have income less than 200%FPL. Benefits cover outpatient services (including outpatient surgery and medications) but not hospitalization. There is sometimes a waiting list. Monthly premium $20 to $70 based on income.

AIT – Addiction Intervention Team – Lutheran Social Services
Trained lay persons will meet with families concerned about a loved one’s drinking, drug use, or gambling. Services will be explained and if the family wants, an intervention will be completed. Women’s Way

The Addiction Center

Faith-based drug recovery centers and programs address a recovering addict’s medical and spiritual needs. The spiritual element is a large focus of treatment, but traditional drug rehab techniques are also crucial. Standard methods like medically assisted detox and behavioral therapy are used to help manage withdrawal symptoms and cravings, respectively.

GYN/Family Planning

AIDS Resource Center of Wisconsin
505 S Dewey St Ste 107, Eau Claire  – 715-836-7710. Will provide free HIV and Hep C (by PCR) testing. Case mangement, psychotherapist, prevention,food pantry for HIV patients. Does not provide Hep C treatment – only testing.

Family Planning Waiver
Provides various family planning services to Wisconsin women and men between the ages of 15 and 44 with income below 200% Federal Poverty Level. Services include: office visits for family planning services; limited lab services; tubal ligation and contraceptives; pap and pelvic exams, diagnosis, and certain follow-up services; STD screening, diagnosis and treatment; HIV screening, and transportation. Once on the program, services can be obtained at any clinic that accepts BadgerCare. However, both the Eau Claire Health Dept (open to Chippewa County residents) and Planned Parenthood not only can sign people up but also provide services on the same day.

Wisconsin Well-Women Program

Community Support Program offers more intensive support to people with significant mental illness but is limited and has a waiting list.
